Patient registration update

We have moved to a new registration system

The University of Michigan School of Dentistry is transitioning to a new electronic health record system. This online registration form is no longer accepting patient registrations.

We will also be transitioning to a new patient portal. A new link will be sent to you so you can complete any required pre-appointment paperwork online ahead of time. Completing the paperwork before your visit will help save you time when you arrive.
